P3.232: Development of a Specialty Fellowship in Pediatric Emergency and Critical Care (PECC) Medicine in Ethiopia

Background: Sub-Saharan Africa faces a significant burden of pediatric critical illness while having few physicians trained in PECC. This project describes experiences and lessons learned through the development and implementation of the first PECC fellowship in Ethiopia at St. Paul’s Hospital Millennium Medical College(SPHMMC).

Methods: A literature search was conducted to understand how other programs developed PECC fellowships in resource-limited contexts. Key hospital and community stakeholders were interviewed for needs assessment. The proposed curriculum underwent a multi-department and external review by the Ministry of Health. The reviews informed the PECC curriculum to align with global partners. The two-year PECC fellowship for pediatric specialists was approved by SPMMC, and six African fellows were recruited in 2024. The curriculum includes virtual and in-person lectures, case discussions, journal clubs, simulations, and bedside teaching by local and international specialists. Oversight subgroups on simulation, point-of-care ultrasound, research, and resource mobilization were created. Educational efforts align with other East African PECC fellowships and the “Pediatric Critical Care in Resource-Limited Settings” multinational hybrid education program.

Results: Lessons Learned: Combining fellowship training in overlapping fields increased interest and reduced the time of training and financial burdens for PECC trainees. Coordinating efforts, resources, and lecturers with regional East African programs increased the pool of pediatric experts available without overburdening faculty. Utilizing established curricula while tailoring education for the local context is crucial to ensure comprehensive and relevant training.

Conclusion: Way Forward: As the first program of its kind in Ethiopia, we aim to serve as a model for expanding PECC training in the region. The PECC fellowship strives to improve healthcare outcomes in Ethiopia and establish a center of excellence in Africa. Fellows are expected to become leaders in PECC and ideally replicate this model throughout Africa. Long-term program sustainability and impact require further evaluation.
